I voluntarily choose to participate in Enlumnia® services.

I understand that Enlumnia provides complementary wellness, spiritual, energetic, intuitive, metaphysical, consciousness-based, educational, coaching, regression, meditation, sound, personal-development, research, training, and related services, modalities, methods, programs, experiences, technologies, and creative offerings, including current and future services developed or offered by Enlumnia.

I understand that Enlumnia services are not substitutes for medical care, psychotherapy, psychiatric care, diagnosis, or other licensed healthcare.

I understand that results and experiences vary and no particular outcome is guaranteed.

I agree to communicate relevant concerns that could affect my participation.

I understand that I can ask questions, request modification of a session, or discontinue participation at any time.

I accept responsibility for my decisions regarding my participation and for seeking appropriate licensed professional care when needed.

For Past Life Regression, hypnosis, guided imagery, readings, and related services, I understand that experiences can be subjective and can include imagery, impressions, memories, emotions, symbolic material, imagination, intuitive information, or other altered-state experiences.

I understand that Enlumnia does not guarantee the historical accuracy or independent verification of regression experiences.

To the fullest extent permitted by applicable law, I accept the ordinary risks associated with voluntary participation and release Enlumnia and Kevin Foresman from claims arising solely from those ordinary risks, except where liability cannot legally be waived.
